About the Role: My client, a growing NDIS Registered service provider, is seeking a dedicated RN Care Manager. You will ensure smooth operations and high-quality care for our participants.

Key Responsibilities:

  • Conduct client assessments and develop care plans.
  • Coordinate medical appointments and community resources.
  • Advocate for clients' rights and quality care.
  • Communicate effectively with clients, families, and healthcare professionals.
  • Mentor and coach junior nursing staff.

Skills & Experience:

  • Registered Nurse with current AHPRA Registration.
  • Experience in care coordination or case management.
  • Strong people management and organisational skills.
  • Excellent communication and interpersonal skills.
  • Knowledge of healthcare regulations and standards.
  • Proficiency in MS Office and CRM systems.
  • Current NDIS Worker Screening and Working with Children Check or willingness to obtain.

Qualifications:

  • Registered Nurse Qualification with current AHPRA Registration
  • Proven experience as a care leader within the health, community or disability industry
  • People management skills including performance management, delegation, follow-up and goal setting with direct reports
  • Previous experience in care coordination, case management, or a related role, preferably in a healthcare or social services setting.
  • Excellent interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse individuals and teams.
  • Strong organisational skills, attention to detail, and the ability to manage multiple tasks and priorities.
  • Empathetic, patient, and compassionate approach when working with clients and their families.
  • Knowledge of relevant regulations and standards related to healthcare and social services.
  • Proficiency in MS Office (MS Word, Excel and PowerPoint)
  • Previous use of CRM systems
  • Excellent time management skills and the ability to prioritize work
  • Current NDIS Worker Screening and Working with Children or willing to obtain (essential)
  • Knowledge of workplace health and safety legislation requirements
