Nurse Consultants provide community-based clinical direction for
patients and clients in their community setting and virtually, forming
deep relationships while delivering unparalleled care to our clients in
their homes. Utilizing our innovative and groundbreaking
Clinic@Hometechnology and your skills and passion for nursing, you
will be a coach, educator, and nurse guiding patients/clients through
their health journey. Every day you will experience the fulfillment of
spending genuine time with your patients/clients in their homes (either
physically or via telehealth), creating meaningful interactions and
supporting telemedicine primary care and behavioral health visits with
providers. Your expertise combined with Clinic@Homewill help you not
only support your client/patient's medical needs but empower them to take
charge of their health.

We are seeking Nurse Consultants who must be comfortable traveling to
clients' homes and interacting with clients via telehealth video visits.
For in-home appointments, we strive to have a 2-hour radius of travel
around our Nurse Consultants homes but final decisions on client
locations will be made after hire. Due to the nature of our business and
growth, other locations may be required. Note: This section changes
depending on where we are hiring and if we are hiring for specific
locations.

This opportunity is best suited for a person who will enjoy
anewadventurous nursing role. Someone technically savvy, enjoys
utilizing technology to drive client engagement and clinical practice
, and is comfortable in a rapidly changing and growing company. The ideal
candidate is passionate about and understands the unique complexities
that working with the underserved Medicaid waiver population and clients
with mental health needs. Finally, this person must be excited to learn
and deploy new and innovative technology to a value-based care model.

Primary Responsibilities

- Manage assigned client panels by engaging patients and caregivers in
patient/client homes and virtually. Educates client, care team, and
caregivers in sub-specialty of medication therapy adherence, vitals
tracking/monitoring, and improving client health. Reviews alerts,
follow up with patients/clients and creates and implements care plans
and goals to improve and maintain client health and independence.

- Supports clients/patients during telemedicine interactions with
providers (either in the home, virtually, or post-visit follow-ups)
to advocate, educate, and coach on success with any care plan
changes.

- Collaborates with telemedicine physicians, pharmacists, nurse
practitioners and home care nurses in Clinic@Home capabilities to
support client needs.

- Drives engagement and independence with the patient/client in the
adoption and utilization of the Clinic@Home platform and hardware
(TV, RPM Devices, etc). Answers questions, provides coaching and
teaching and helps problem solve.

- Assess potential clients for Clinic@Home during consultation visits
and support new clients during Activation visits. This includes
educating new clients on the software, remote monitoring devices,
assigning health education tools to help the client meet their
health goals.

- Collaborates with case managers, care managers, providers, and other
third-party care team members to ensure patient/client needs are
being met.

- Works in the EMR and CRM to ensure timely and accurate clinical and
business documentation.

- Thinks outside of the box for strategic organizational solutions and
during coaching for clients.

- Maintains education and competencies to remain proficient to provide
high-quality patient care practice and skills.

Required Education

- Associate Degree or Bachelor's Degree in Nursing

Required Qualifications

- Current, unrestricted MN State RN License

- BLS Certification

- At least 2 years of nursing experience in the following areas: home
care, mental health, community-based nursing, skilled nursing
facility, public health, visiting nursing, and/or private duty
nursing (PDN).

- At least 1 year experience working with underserved populations such
as those on the Medicaid waiver program or other socio-economically
challenged populations.

- Ability to remain calm and composed under pressure, ability to
troubleshoot in unexpected situations.

- Comfortable working in clients homes and with clients and
caregivers with multiple, complex, and chronic health conditions
including behavioral health.

- Computer proficiency (Microsoft Office Suite, Google Suite, etc) and
EHR/EMR experience.

- Ability to learn new technology quickly and proficiently

- Interdisciplinary team experience and ability to collaborate and
communicate with all members of the healthcare team (Homecare nurse,
IHS, PCA, case manager, healthcare specialties)

- Excellent oral, professional presentation, and written skills are a
needed plus must have the ability to work independently 90% of the
time.

- Must be able to organize your workflow of daily tasks and be
able to complete them in an efficient time.

Preferred Qualifications

- Medication set-up or administration experience

- Comfortable working with clients and their caregivers in their
homes with multiple, complex, and chronic health conditions
including behavioral health

- Remaining calm and composed under pressure, ability to troubleshoot
in unexpected situations.

- An understanding of the promotion of health and wellness and
enhancement of an individuals ability to live independently in the
community.

Typical Work Schedule:

This key role is performed in a hybrid environment. Nurse Consultants
work primarily in clients private homes/group home settings and
virtually (either from home if a private office space is available or at
our office in Minnetonka). Work may also need to be conducted in your
vehicle or other location while traveling. Candidates must havereliable
transportation and enjoy a mix of working from home/office and
traveling to multiple client home locations daily. Occasionally travel
outside the metro, in Minnesota, is required.

The daily schedule is Monday - Friday 8am-5 pm.
