As a Physical Therapist in home health, you will:

• Conduct comprehensive physical assessments and review medical histories to evaluate patients' conditions.
• Develop and implement individualized treatment plans tailored to each patient's unique needs and goals.
• Administer therapeutic exercises and training programs designed to maximize functional independence.
• Educate patients and their families on injury prevention, wellness, and ongoing health management.
• Monitor, document, and communicate patients' progress toward their rehabilitation goals.
• Collaborate with a multidisciplinary team to ensure a comprehensive approach to patient care.
• Maintain accurate and timely documentation in compliance with healthcare regulations.

Qualifications:

• Masters degree in physical therapy.
• At least one year of experience as a physical therapist in a home health setting.
• Current state licensure to practice as a physical therapist.
• Compliance with accepted professional standards and practices.
• Valid CPR certification for clinical roles.
• Valid driver's license with the ability to travel to various locations as needed.
